Early Intervention Support Services - EISSThe Wellness and Recovery Center at Saint Clare’s Hospital is a crisis intervention program designed to work with individuals experiencing distressing symptoms who are at risk of utilizing emergency services. The Center offers an alternative to providing care for a person experiencing a crisis situation in a setting that is separate from the medical facility. A multidisciplinary team provides services for a period of time that varies based on the individual. For some, one or two visits are sufficient, while for others the maximum enrollment period of 30 days is beneficial. In addition to crisis intervention, services may include supportive counseling, case management, substance abuse assessment, medication evaluation, peer support, and family intervention. The Wellness and Recovery Center ensures a timely response to individuals in need of crisis support while promoting the principles of wellness and recovery. Through early intervention, risk factors may be drastically reduced and the development of more serious problems lessened. Appropriate linkage is coordinated based on a person’s needs and preferences, as well as available resources. A main focus is to begin discharge planning with individuals as soon as possible so that a seamless transition is ensured. Referrals may be made to the Wellness and Recovery Center by the individual in need, a provider, a family member, or any other concerned member of the community. The person may be seen in the community or in the welcoming environment of the Center. Individuals 18 years of age or older who are residents of Morris County are eligible for services.
