Community Based Program

General Information
Description of ServiceAn intensive, short-term emergency intervention and education program for families experiencing child-focused crisis where one or more of their children are at imminent risk of being placed out of home. FPS provides a minimum of five to eight hours of face-to-face intervention per week for a length of four to six weeks. The goal of the program is to prevent placement if a child can be safely maintained in the home. FPS can also be utilized to assist in the transition of a child returning from placement to their home.

Criteria for referral:
1. Child can be safely maintained in the home.
2. Less intensive services have exhausted, unavailable or deemed inappropriate.
3. At least on primary care giver is willing to work with FPS and agrees not to pursue placement during the intervention.
4. Alcohol and substance abuse do not interfere with appropriate participation in the program.
5. A child is returning from placement within the next four weeks.
