Description of ProgramIntegrated Case Management Program case managers provide a minimum of eighteen months of aftercare services to all individuals leaving the State Psychiatric Hospital and to some clients leaving the Atlantic City Medical Center. The program counselors assist consumers in successfully reintegrating into the community.
